Coverage requires that the etiology of Growth Hormone Deficiency (GHD) is a result of destructive hypothalamic or pituitary disease, radiation therapy, surgery or trauma OR is a result of documented GHD in childhood.
Additionally, a Growth Hormone Stimulation test (which requires hospitalization) to verify your pituitary gland has basically ceased to function needs to be conducted by an Endocrinologist.
